Songs For Kids Mentor Application

    All Songs For Kids Musicians undergo a thorough screening and training process to protect the kids we serve and to make sure they have a great time. If you move forward in our application process, you can expect to complete the following requirements:

    • Complete a background screening
    • Participate in training with Songs For Kids musicians
    You may also be required to complete a medical screening, which may include a TB test and annual flu shot, and COVID-19 vaccination. To get started, please fill out the below application and someone from Songs For Kids will be in contact shortly!

    Please note that this application must be completed in its entirety in one sitting.  Please make sure you have the following available before you begin:

    • Two references and their contact information
    • Your specific schedule availability (by day)
    Please describe your musical background and provide relevant details for any teaching experience and prior work with kids:

    Please supply information for two references, preferably former or current co-workers or supervisors
     
     

    Availability

    Please check off each day and time range during which you generally expect to have availability, even if it's not the entire range. For example: if you can usually do evenings 5:00-7:00pm on Monday, please check the evening box.
